Our Services: Fostering Independence
Individual Residential Services and Supports (IRSS)
Provides 24/7 daily support to persons with disabilities living in a Personal Care Alternative in their own home, a home owned by the agency or a host home.
Supported Community Connections (SCC)
Provide support to individuals with disabilities opportunities to build relationships and natural support in the community.
Community Connector (CC)
Wells Healthcare Foundation Agency LLC will provide CC services to individuals in the community. CC services are designed to support individuals with opportunities to build relationships and natural supports in the community, interact with

Non-Medical Transportation is a covered HCBS waiver service that assists individuals in traveling to and from waiver-approved services and community activities when they cannot safely or independently use public transportation.
